• Names are released for one-time rental only

• Names are for direct mail purposes only. No telephone, fax numbers, or e-mail addresses are available.

• For rush orders less than 7 days add $25.

• List may not be sold to another vendor.

• A copy of the mailing piece must accompany order.

• Pre-payment is required for all orders. Payments may be made by credit card or check in the appropriate amount made out to LCA must accompany order.

• Labels are available in a peel and stick format in zip code order.

• Available as email attachment in Excel format if the statewide list is ordered.
